    Default Name Change for Minor in Texas

    My question involves name change laws in the State of Tx: I have a 5 yr old and she has her father's last name. She hasn't seen or heard from him in about 4 yrs. He and I have agreed that I could add my last name to hers. How would I go about doing that?

    The relevant statute about changing kiddo's name is right here.

    If Dad agrees it should be a piece of cake - call the court-house, ask the clerk which forms you need. The clerk can give you that information (but s/he won't be able to tell you how to fill them out).

    Your county may even have the forms online!
