Default Adoption when Putative Father Lives in a Different State

My question involves adoption law for the State of: Arizona/Ohio/Indiana

My daughter is almost 3yrs old. her biological father lives in Indiana. He has not seen her since November of 2007, has only sent her one christmas card with a $30 gift card (this is the only "money" he has ever given her) and his name is not on the birth certificate. She was born in Ohio. We now live in Arizona with my husband of 7 months. my questions are what state do i need to start the adoption process in? is the status of her relationship with her biological father considerd abondonment since he has not seen her or supported her?

You would do so where the child lives, so Arizona.

Since you know where to find him, probably not. You'll likely need to get him involved in the process, whether he's on the birth certificate or not.
