Are you asking as a landlord or tenant? As a tenant who wants to know if you can make modifications at your own expense, or compel the landlord to pay? As a landlord who wants to know what he has to pay for, and when he can say "no"? For an existing unit or new construction?
I'm asking for my parent's as tenants. They want to know if they are allowed under the ADA to put grab bars in the shower/tub. And then they want to know if they have to pay the full cost or if the landlord needs to pay any of installation. My parents moved into this apartment in mid December. Have never had a landlord before. They have already told me some things the landlord told them that contradicts with ADA law - such as they can't have a service dog - so they are worried what will happen if they modify the bathroom.

I think the ADA makes a clear distinction between a landlord renting a property to be used by the general public in a general commercial setting versus one used by a private party (i.e. a residence)...
