One employee makes mean comments about you. You're concerned that others may not like you. So far it sounds like junior high school. From what you say, your supervisor has been receptive to all of your complaints.

Apparently, my Supervisor was never informed of the incident and his boss never followed through on the incident. I told my supervisor I would call tomorrow on whether or not I put my notice in but I needed to be evaluated by a doctor.
Your supervisor was never informed of an incident that you reported to your supervisor? That doesn't make sense. If you mean you chose not to share a full account with the supervisor when you made your report, why did you make that choice? Your employer can only respond to things that they know about.
