Default Seeking Paternity Determination and Custody

I am getting a paternity test to see if the supposed son of mine that I have been taking care of for the past year is mine. We chose not to put my name on the birth certificate until we found out the out come of the test. We are broke up now and she will not let me see my son. I will be taking the test in 2 days with him. She is going to let me visit long enough to do this. She will not change his last name to mine if he is mine and she will not put my name on his birth certificate if he is mine. What can I do to get my name on the birth certificate if he is mine and change his last name. I want full custody of him because she is not taking care of him the way she should be. He had a diaper rash so bad last time I seen him that he had open sores. He usually has diaper rash whenever I see him, its not as bad but it still hurts him. He has diareah most of the time because usually all they give him all day is liquids and cheerios. She is blaming the diaper rash on me but I have witnesses that know that it isnt from me. I know that when this goes to court she is going to be a pain about it she already is. I need to know what kind of rights I have and any info on taking full custody if he is mine. I am driving myself crazy trying to deal with this and she is not helping me. This is hurting me and my son not being able to see each other. PLEASE HELP ME FIGURE THIS OUT!!!!!!

You and the mother can establish your parental status by completing and filing a voluntary acknowledgement of paternity, or you can file a paternity action and seek a court order declaring you the child's father. Once you are legally the child's father you can get your name placed on the birth certificate, and petition for visitation or custody.
