Default Inventor's Name Not Showing Up in a Patent Search

I had a former employer inadvertently leave my name off of a patent application that was filed after I left the company. I found the patent application while searching the uspto.gov website. I contacted the employer, and filled out all the necessary legal forms to have my name added to the patent application as an inventor, and I'm able to see that the documents were appropriately filed (via the PAIR website). This was a few years ago, and the patent has subsequently been issued, but why is it that my name does not show up in the uspto.gov search system as an inventor?

In terms of correcting the patent to add an inventor, read this. Were proper procedures fully followed?
