Default Help With Letter from Collection Agency

My wife received a letter from collection agency working behalf of Gold's gym saying she owe's $$$$$. The gym had told her that a doctor certificate was all thats needed for her to cancel the membership. She did not sign the contract, since doctor's certificate was not mentioned in the contract form. But she did sign for EFT (which is in the same contract form, but under different section). Does this mean she has signed the contract ?

Default Debt from Contract with Gym

If your wife authorized payment for her membership, it seems likely that she signed the contract. If you're not sure, have a lawyer in your state review the materials.
