Default Visa Denied Even with an Approved I-212

From my understanding, an approved I-212 allows the person to re-enter into the U.S. Is it possible for the US Emabssy officer to deny a visa with an approved I-212? Is it legal to ignore the approval?

Having an approved I-212 doesn't trump other immigration laws.
