    Default Authorization to Release Information

    My question involves an injury that occurred in the state of: NC.

    My wife was in an auto accident last year and was not at fault. The insurance company has sent an "Authorization to Release and Disclose Protected Health Insurance Information". This looks to be an open-ended form allowing access to all of her medical history. Is this normal or should we ask for a more specific forms targeting certain dates?

    It's normal, and is typically used only for records relating to the accident.

    (At least, this was the case when my husband was hit by a car a couple years ago and we received similar forms.)

    You'd do well to consult an attorney to ensure that they don't go after more records than they strictly need, though.
