    Default Hyphenating Daughter's Last Name

My question involves name change laws in the State of: California

I would like to Hyphanate my daughters last name with my and my husbands
(her stepdads). I am willing to try to get permission from my daughters biological father. Should he say yes I would like to know how to go about it right away, before he has second thoughts. What I need to know is what forms are needed, and weather or not court will be needed for such an agreed change in name. Thank you for your time.
