My question involves name change laws in the State of: FLORIDA.

*LAST NAMES HAVE BEEN CHANGED FOR PRIVACY.

My last name is Gosselin.
My stepdad & mom's last name is Edwards.
My fiance's last name is Williams.
My ex (son's dad)'s last name is Webber.

At birth, I didnt let my ex sign the birth certificate or give my son his last name. I gave my son my stepdad & mom's last name because once I turned 18, I wanted to change mine as well because my biological dad isn't around. Medicaid puts my son's name at the doctor's office & hospital as Jacob Gosselin-Edwards. That doesn't make sense to me. Please explain that. Plus, I never changed my last name to Edwards because I'm engaged to be married this year to a man with the last name Williams. I'm in the middle of court & mediation with my ex & once that's all settled, I have the choose to give my son the last name Edwards-Webber or just Webber. I want my real last name now as my son's last name so he would be named Jacob Gosselin. & if I HAD to, I would add the Webber to that hyphenated. Can this be done? What are my choices? Do I have to pay for my son to have my ex's last name since I was the one who wouldn't let him sign the birth certificate in the first place??
