Default How to Hyphenate My Teen's Name

My question involves name change laws in the State of:Texas

My daughter is 14. The father is unstable and owes her over $40,000 in Child support.
We have a really good relationship, and she realizes, that he was there for 14 minutes, while I've been here every minute of every day, every crisis, every success, every emergency, every bill. I've covered her with health insurance and I'm paying for the tutor, soccer and the braces. I'll pay for college and the car. I'm there when she's sick, happy, sad, it's mommy. always mommy.
It makes me ill that she carries his name instead of mine alone, but I will not disrupt her life. what we've discussed and are excited about, is hyphenating her last name, to add mine.
    what steps do I take?

With dad's consent, you go through a consensual name change process at your local courthouse. Without dad's consent you litigate the issue, and attempt to prove the name change is in your child's best interest, normally through the custody court.

You can ask at the court to see if they can provide a standard name change petition for a minor, along with instructions.
