My question involves name change laws in the State of: Oklahoma.
My daughter is 15 years old. For the past 6 years she has lived with me and her step-father and her siblings. Her biological father has not paid a dime in child support in 11 years and he has not seen her in 4 years. I'm not even sure where he lives anymore, although I know it is somewhere in the state. She wants to change her last name from him to that of my husband's and her siblings. I am planning on doing this on my own and have all of the forms I should need to do so but I'm not sure about notifying him.
Do I have to have him served or notified directly in this situation since we really don't even know where he is and he has nothing to do with her or can we just use the publication method of notice?
Also, can a legal name change be used to change her last name on her birth certificate or do we have to do a full blown adoption in order to do that?
