Does the judgment require that her name come off the mortgage or not? If not, does it state that her name will remain on the loan, or is it silent about that while requiring that you refinance the property (which implicitly would involve taking her name off the mortgage)?

In terms of your general question, no, if there's an ambiguity in a judgment then one of the parties is entitled to have the court clarify it. You have the right to argue that there's no ambiguity.
