    Default How to Add Mother's Maiden Name in Pennsylvania to a Child Who Lives Abroad

    My question involves name change laws in the State of Pennsylvania.

    My son was born in Philadelphia, PA, in 2002. He is an american citizen, althoug his parents are not. None of them is living in US any more.

    The three of them (child, mother and father) would like to add the motherīs maiden name to the child last name (the child last name is his fatherīs last name).

    How can we do this legally? Should we do anything before the Pennsylvania Department of Health, Division of Vital Records?

    The point of this process being what?

    If they get a legal name change in the nation where they're presently located, they may be able to get the Department of Health to change the birth record based upon submission of proof of the name change; or they may have to hire a lawyer in Pennsylvania to register the foreign judgment for enforcement. I don't know enough about the context, or the foreign nation involved, to say more.
