    Default How to Add Mother's Maiden Name Legally in Philadelphia County

    My question involves name change laws in the State of: Pennsylvania

    I'm an adult living in Philadelphia, PA. I would like to add my mother's maiden name to my last name. I have never been married and I have my biological father's last name. For example, I want my name to included both last names...Name Change-Request. Do I need to take legal steps to do this or can I just add the name and change all documents (Driver's license...)?

    If you want the name change to be legal, you must file a Petition with the Court.

    For Philadelphia, go to this page, and scroll down to "01-101A Name Change Petition Package", then download that package (it the forms you need to change your name).
