Default How to Get Bio Fathers Name Off Birth Cert. in Wisconsin

My question involves name change laws in the State of: Wisconsin
I was wondering if someone could give me advice on how to go about getting a dead beat dads name off of a birth certificate, and how to change my son's last name from his to mine. Out of the 7 years of his life he has been involved in the first 6 months. I have never taken him to court for support or anything but I just want to chage my son's name. Thanks in advance!

Go there. Read through it, and a link to the forms is provided on the page.

If he is on the birth certificate and signed the AOP, he is the father. Unless you have a husband of at least a year to adopt the child, he is going to remain on the birth certificate as the father.

If you would like to change your child's last name, you will need his dad's permissions to change the name.
