Default Getting Father Added to an Adult's Birth Certificate

OK, this is very complicated. My father was married to a women lets say named Lou. He had an affair with her younger sister (Ann) & she got pregnant. Lou & my father split up for a while but before I was born Lou and my father got back together. Ann was so mad that she did not list my fathers name on my birth certificate thinking that would make him come back to her. When that didn’t work she decided that she didn’t want me & gave me to Lou and my father to raise & than she left the state. My Dad & Mom (Lou) raised me. All the years I always thought she was my birth mom & she never once showed me any different. She was & still is the best & I thank God everyday it was her that chose me. Anyway, they had me baptized at a very early age using a birth announcement with Lou’s name listed as the mother. I guess back in the late 60’s they didn’t ask for much proof. With a baptism record being just as good as a birth certificate, they were able to raise me with my father’s last name. EVERYTHING I have ever had legal has been with his name. (Other than when I was married, but have now gone back to my fathers name)
Through the years I have lost the Baptism record & that church has long ago burned down so I can’t even get a copy of that. My name is also listed as the mother on my daughter’s birth certificate. So, my problem is, my daughter is a senior this year and I was planning on taking her & a friend out of the country after graduation.
(Next June) I haven’t seen my daughters birth certificate in years, (Packed away in some box) so I thought I would order her one. In doing so, I thought I would order myself one also, so I can go ahead & begin the process of getting us a pass port. Well to my surprise I don’t have a birth certificate, as my parents listed. My parents are divorced now but have stayed friends. I called my Mom to ask her if she had a copy of mine still & than I started asking why they couldn’t locate mine. She hurried me off the phone & said she would look in some boxes over the weekend & let me know. Well I received a 3 way phone call from both my parents when they knew my daughter was not home & they told me the story. I was crushed & decided at that moment I NEVER wanted my daughter to know the truth. So now I am trying to find a way to get my fathers name added on my birth certificate because it has Ann listed as my mother & father unknown. How can I get my Dad’s name added so I at least have some kinda
Proof why I was raised with his last name when nothing legal was ever done to give it to me. And also, what steps would I have to take to get my Mom (Lou) to adopt me after my Dads name is listed, so I would have a birth certificate stating what I already know in my heart?
I am in Florida, But both of my parents are in two other states.

Thanks Aaron for asking, But after days of searching I do believe we have found
    the answers.

I was born in California, so I know everything has to go through the state, even though I am now in Florida. I have ordered the papers that I need, since they told me I needed original's not copies) My Mom still lives in Cali & my dad & birth mother live in Ohio. I have to send the form to my Dad & birth mother to sign & notarize and send all the forms along with copies of all of our birth certificate's. I did get my birth Mother to agree to add my Dad's name. California told me it would take about 7 months.

Once his name is added on my Mom is going to legally adopt me. At that time they will issue a new birth certificate with my Dad & Mom (The women that raised me) as my parents. And the records are sealed. Also since my Mom is in California & doing it there, we do not need permission from my birth mother to do this. (She refused to ever allow my Mom to adopt me)

If you have any advice, or know of any other way to make this easier, please let me know.

    Thanks again.
