    Default Medical Records

    I have been asked by Social Security to prove my disability prior to my 21st birthday. I am 46 yrs of age. The hospital that treated me prior to my 21st is closed down. What do I do? Is there a source where patient records are kept? Archives? Would appreciate your response. Thanks.

    Default Old Medical Records

    If the hospital was part of a larger organization or chain, that organization may still have records.

    If you were referred to or from the hospital by outside doctors or medical practices, those doctors or medical practices may have records which would include materials sent to them by the hospital.
