Default Changing Childs Name to Father's Last Name

My question involves name change laws in the State of: Florida
When the child was born the mother was not sure who the father of her child was and therefore gave the baby her last name. Upon determinantion of paternity I assumed responsibility for my child. However I want to correct the birth records, and change my childs last name to my last name. Her mother objects. I sought and paid for the paternity test for peace of mind and take my parental duties seriously as a father. How do I go about/what are the laws as to changing a childs name to the biological and socially/financially responsible father?

And, if you can convince the mother to agree to the name change, forms are here.
