    Default Voluntary Employee Wage Assignment

My question involves employment and labor law for the state of: Illinois
Regarding a voluntary employee wage assignment. does anybody have a standard template for this using the appropriate language and terms?

A garnishment debt originally imposed on the employee has been shifted to the employer. We end up being responsible for the employee not paying rent for 6 months, because we failed to respond to a motion (longer story than is necessary here....)
I understand the wage assigment is voluntary, but I want to pursue it anyways and want the right language to use.

Thank you for any help with a wage assignment form, format, etc.

Your clarification has left me confused.

    Is there a garnishment now?

Does the employee want you to take money out of his paycheck?

No, but we want to try to recoup some of it from him through the wage assignment.

The original garnishment from the landlord was never put in place by us and the debt must now be paid by us, the employer.

There is no garnishment, I am assuming.

So draw up an agreement in plain, clear, and simple English (unlike what you have used in your post) about what y'all agree on and then take the dough (after both y'all sign it).
