insurance companies are not liable for anything other than what they are contracted to their clients for.

    a. If she gets into an accident, will her insurance be liable or mine?
per your contract, SHE will be liable. If her insurance is contracted to pay for her debts for however the situation happens, they will pay. If they don;t, she will still be liable.

    b. If she gets into an accident and the other party involved decides to file suit, who is liable for any costs that are not covered by insurance?
chances are the both of you would get sued and the courts would decide but unless you knew she shouldn't have been driving or there was something within your control that was contributory to the accident, she should be held responsible for any additional.

Have her add you as an additional insured. If the truck is damaged they will pay her and not you unless they are notified that you actually own the vehicle and put you on the policy.
