Default Credit Bureaus And Medical Bill Collection

I got an email alert that there was a change on my file. It was a collection which I truly do not believe is mine. It is for $633 and I got the info about it and called both NCO Finance and AR Gatewood. Ar Gatewood is the one that turned it in to NCO Finance. When I called they said that I had entered my insurance provider as something called Qual Insurance. This is when I was quite certain it was not bill because I had never heard of that insurance company. It would be quite impossible for me to enter a company I did not know existed. So....I sent certified letters asking for proof. I specifically stated that I wanted a copy of the original invoice on the original letterhead, with my signature and the date I signed it. They sent me back an invoice with my name typed on it. I called them and told them that anyone with any computer knowledge could have typed up that invoice and I wanted to see my signature. They could ot come up with it and they removed the collection. That was in December 2007. Now just two months later, they have placed it back on my file.
Can anyone tell me what I can do? They just flat ignored the Fair Isaac Credit reporting act. My credit score has not been less than 721 in years and most of the time, it has been 750 - 793. I have always tried to pay my bills early and more than I owe to keep my credit score in good standing.
Can I sue the credit bureau? Is there any attorneys out there that will do it on a contingency?

Send the certified letters again, return receipt requested. Note that the account is not yours. Demand that they provide you with the information requested, including the SSN on the account (don't offer yours) and demand that if they are unable to provide the proof you require, they remove all traces of this from your credit report. They need to prove that it is your debt, not the other way around.

Then dispute the debt with all three credit reporting agencies, noting that the debt is not yours and that the collection agency refuses to provide proof otherwise. Additionally, file a complaint with your state's attorney general.

This is not the fault of the credit bureau, but rather of NCO.
