    Default Name Change In Florida

    REcently my finace and I got married. My daughter is 9 years old and my current husband is the only father she has known. I do not know who her "biological" father is and I am the only parent listed on her birth certificate. Due to my recent marriage my daughter desperately wants her last name changed to my married name (my current husbands name). What steps are involved in this and is is fairly easy or common for a childs last name to be changed?

    Hi Fitzhuc!

    Since your daughter's biological father is not listed on her birth certificate, you won't have to worry about getting his permission.

    Fill out
    FLORIDA SUPREME COURT APPROVED FAMILY LAW FORM 12.982(c),
    PETITION FOR CHANGE OF NAME (MINOR CHILD(REN))


    and

    FLORIDA SUPREME COURT APPROVED FAMILY LAW FORM 12.982(d), CONSENT FOR CHANGE OF NAME (MINOR CHILD(REN))

    You'll need to contact the clerk of the Circuit Court in the county in which you live to learn about fees and other procedures.
