    Default Changing A Child's Name

    I live in Washington state and when my 5 year old was born I gave him two last names, mine and his fathers. I now want to change his last name to just his fathers and am wondering how I go about doing that. I appreciate any help!

    You should be able to get a name change form from the District Court in your county. The process is described here (PDF).
