    Default Adding Father To the Birth Certificate And Taking His Surname

    My step-daughter, that is now 20, wants to add her father to her birth certificate and take his sur name. His name was withheld at the time of her birth. We did a paturnity test, which we have the results to. She lives in Oregon with us, but she was born in California.

    I have spent the last hour getting ping ponged around from vital records and other state offices tring to find out how to accomplish this.

    I would like to know if we can do it in the Oregon courts or will it have to be filed in California. What documents and paper work will be needed. Oh and were to get it.

    Contact California's Office of Vital Records, and find out if they will accept an acknowledgment of paternity (PDF) executed past the child's eighteenth birthday.
