    Default Notary Public Wording Problem

I'm new to the forum. I need advice on how to word the part of an agreement or contract where a Notary Public signs and seals. This is in the state of MI. I may not be in the right forum to ask this question, but I'm rushed for time. It is a contract regarding terms for the transfer of personnal real property for use as a 501(c)3 org. It is a personal contract between myself and the owners, and they have asked me to draw it up. I'm good, untill I get to the notery public part.

This is what I believe I need:

The foregoing instrument was acknowledged befor me this_________(date) by____________&_____________.
    __________________________

    My Commission Expires
    ___________________

    Should anything else be included?
