Exclamation Help Adding The Father's Name On The Birth Certificate

My daughter was born in New York, she's 9. Her father was not present for the birth so his name is not on the birth certificate (we are not married and never were). We would like her father's name on the birth certificate (and her father agrees). However, he is currently an inmate at a New Jersey department of correctons. I told to get an Acknowledgement of Paternity form and have him to compete his section.

My question is, in New York, do I need to obtain an attorney to have my 9 year daughter's father put on her birth certificate? Thank you.

    Please Help!!!

You don't need a lawyer to complete and file an acknowledgement of paternity.
