Minor thrown out by her family

My husbands niece is pregnant 15 and her mother never had anything to do with her and has a homeless drug infested ife, and her father threw her out when he found out she was pregnant. My delema is that she wants to go to school still, instead of running to her "boyfriend" she made a choice to come to her family. Her father will not give her anything, legal info such as social security number, birth certificate, or clothes. Therefore, I have a problem, she needs to be under dr care and cannot get medicaid, (no legal info), and the school says we need to get a signed form from he rfather saying he gives us permission to take responsibility. However, keep in mind, he does not want to cooperate in any way to help her.
What am I going to do?
