    Question How to Authorize a Temporary Caregiver to Obtain Medical Care for a Child

    My question involves a child custody case from the State of: CT and GA

    My kids, husband and I are from GA. I'll be having major surgery in a few weeks, so we're bringing the kids to family friends to care for them while I recover. The family friends live in CT.

    I'm trying to determine what forms we need to provide the friends to be able to take our kids to the doctor if needed or to show proof they are their temporary caregivers.

    I'm assuming it's a form based in CT since that's where the kids would be located, but I'm not sure.

    If this is in the wrong spot, please let me know the appropriate place.

    A Power of Attorney (POA) is what I always used in similar circumstances when my child was young. No need for temporary custody or anything of that nature.

    When we left our kids with family out of state due to an overseas trip, I spoke with (and filled out some forms - no longer remember what) the pediatrician my parents would go to, as well as the local hospital.

    Ask your pediatrician's office if they have a form. There is at least one generic form online. Be sure to attach a copy of the child's insurance card and your local ped's contact info.

    Local ped is in GA, the kids will be in CT.

    And surely the folks in CT know a local pediatrician. Call them and find out what documentation they would need.
