If a U.S. citizen marries his fiancée while she is in the U.S. as a tourist, will that affect their later ability to get a spouse visa for her? Obviously, she would be returning home after the wedding.

Would it create problems? Probably not. The mere act of getting married in a country is not evidence of intent to immigrate, and a return home without overstay would reinforce the idea that the entry was made without intent to immigrate.
The wrinkle is that you're suggesting that there would, in fact, be a plan for future immigration, and that the plan would not be disclosed when seeking the tourist visa (as disclosure would be disqualifying). I wouldn't expect that deep of an inquiry, but if you want to best prepare for all questions and outcomes it makes sense to consult an immigration lawyer.
