I was not investigated I was part of investigating. That's why it came as a total shock.
Ok so they see it as you did not treat the in incident with enough seriousness and escalate the matter to a higher authority.

They told me while on leave I failed to escalate a client situation.
The investigation of
incident itself was completed before you went on leave. It appears the investigation of your actions during the investigation apparently did not conclude until after you left on leave.

And disability as diagnosed numerous times is mental with bi polar and depression
so where do you believe the diagnosis gets you? If you want to talk about fmla; you are being urged to leave and or fired because of your actions at work, not for taking leave.

Ada; what accommodations have you sought that were denied? They do not have to accept substandard work because you are bi-polar. Unless successfully treated, there is nothing inproper about terminating you because of being bipolar. You are still expected to maintain the same standards as every other employee.
