    Question Uncontested Name Change Procedure in California

    I am planning on changing my daughter's last name from my maiden name to my married name. She has lived with her stepfather since she was 7 months old, she just turned 8. It is her choice to have her name be like his and her sisters.

    Her bio. has no objection, which is great. Where I am lost is with all the paperwork. We are filing jointly so I don't have to serve him, but we have 2 different addresses/phone numbers. Whose do I put at the top? Do I file the whole packet together at first, or just the Order to Show Cause? And after I file, what happens? The courts won't answer my questions and the family court lawyer I contacted said they don't assist with name changes.

    Just follow the instructions.

    Fill out everything the best you can and in writing ask for a hearing date and make sure everyone shows up.
