My question involves paternity law for the State of: Louisiana

My sister is still legally married to (did not change her last name) but has no contact with the father of her 4 year old daughter. He left them when her daughter was 2 months old and has not had any contact with them in over 3 years. He has been in and out of jail during this time. Now that the child is about to enter school, her mother would like to either change her name to match hers and remove the father's name, or at least hyphenate the last name. Is this possible? How should she proceed? Please advise.
Her husband is the legal father? He'll have to be served. She should check her local parish to see if they have forms online or if she has to go in person to pick up the forms. But yes, he'll need to be part of the equation in some way.
