Default Citing Legal Authority for Claimed Bankruptcy Exemptions

My question involves bankruptcy in the state of: FLORIDA

Could someone please let me know what I should put or if what I have put in the following circled fields is correct?
Should I have checked that top or bottom box? I've also looked everywhere for the specific laws that allow exemption... Florida does not accept the Federal exemption.

Now on the Unsecured Claims... could someone please let me know what I should put or if what I have put in the following circled fields is correct?
The top one is for a student loan (I understand this is non-dischargeable).
Second one is for a credit card which I also have a checking account with BOA... is it subject to offset?

One last question please on the creditor matrix text file that I need to submit. Should I be including account numbers?

Much appreciated, thank you in advance.

You can find Florida's exemptions in Chapter 222 of Florida's statutes.

You will need to read your BOA contracts.

As a general rule, account numbers are not required on a mailing matrix, and should you include account information you would identify accounts only by the last four digits of the account number in order to keep sensitive information out of what will become a public document. Some local rules prohibit including account numbers, so you should check with the court in which you'll be filing to determine its policies. For example, here's the guidance offered by the Bankruptcy Court for the Southern District of Florida.

Thank you sir! One last thing... on the last image I posted, should I be checking the box for "Contingent", "Unliquidated" or "Disputed" for personal unsecured credit card debts?

A credit card debt would not normally be contingent; I don't see any reason why it wouldn't be possible to determine the exact amount of a credit card debt; and you haven't told us of any dispute.
