    Question How to Change Your Last Name on Your Birth Certificate

    My question involves name change laws in the State of: Tennessee

    My birth certificate has both my mom and dad's last names. I want to permanently take his off.

    How do I go about taking my dad's last name off of my birth certificate?
    The birth certificate is from the state I grew up in, is that going to matter when I want to change it?

    You can't.

    If you're an adult you can certainly change your own name, but you can't erase your father's information.

    You may not like your father, but you cannot change his name for him or erase his name. Like it or not, the birth certificate shows the parents and that includes their first and last name at the time of birth. You cannot change that fact. You may change your own name, of course, but it still won't change how your father's name appears on the birth certificate.

    I know I can't take off my mother and father from my birth certificate. That's not what I'm asking, What I am asking is that both of their last names are on my name.

    I have my first and middle name, and then both of their last names on my name. For example...

    John Anthony Smith Forbes. --- The two last names in this case being Smith and Forbes. On my birth certificate, my dads last name is where the smith is... in this example.

    That's straight forward. You go to court and have your name legally changed.

    One thing I'd recommend is to immediately go to the clerk and get a number of "true copies" of the order (these will typically be embossed). You'll have to show them (and possibly leave a copy with) various other entities to prove you legally changed your name. Same thing with marriage certificates when you get married. I started with a half a dozen and I think I only have one left.

    Well, I also did get married last year so I guess I have double the work to do.
