Thanks everyone for your assistance. The issue is resolved.

Supervisor shredded the disciplinary form after realizing I wasn't lying. I mean honestly, what would I have to gain by not reporting to work. I know HR is there for the company and employees are irrelevant; nonetheless, employees "should" be treated equally but most are not due to family involvement or favoritism. Anyways, he/she apologized to me and problem is gone.
