    Default Correcting a Name on an Adult's Birth Certificate

    For the past year I have been sending various documentation ( from immunization records to doctors let ters,official high school transcripts, job ID and drivers license) to the city of New York (born in Kings County Hospital brooklyn nyc) to have my first name added to my birth certificate and each time my application has been denied I really need to get this done as soon as possible as I have various health issues that will get worse.
    I believe at this time i need legal representation to have my first name added to my birth certificate (in order to get my passport to visit my family before my health deteriorates)

    But this appears to show me how to change name in court: I'm trying to add my name on my birth certificate, which was left off since i was born.

    If it goes beyond a simple error and fixed by this: http://www.nycourts.gov/courthelp/Na...tificate.shtml

    You will need to get a court ordered name change to change it.

    if the name was left off the birth certificate, then you have not been using your given name all these years. You either get to officially change your name from what your given name was to what you have been using or not. You cannot correct a birth certificate that is not considered to be in error.
