    Default Adding the Mother's Surname to a Child's Name

    When my daughter was born we put her fathers last name on the birth certificate. I don't want to change her last name, but i would however like to add mine as well. Would this be a possibility? And is this something that would be difficult to do?

    If the father agrees with your plan, it shouldn't be a big deal. You chose not to follow the instruction to identify your state, but it should be easy enough for you to find name change information for your state.
