My question involves unemployment benefits for the state of: New York
I'm new to the forum. I have read some similar posts but can't find answers to some specific questions.

Here is my situation:
I'm collecting NY unemployment benefits and got a temporary, short project as an Independent Contractor with a company in CA.
For the week ending 06/29/14 and 07/06/14, I reported the days that I worked (2-3 days) and received partial benefits. For the week ending 07/13/14, since I worked 4 days, I called Telephone Claim Center and was advised to skip the claim for that week. I finished my work on 07/11/14.
The project is currently reviewed by the company's client. The company will contact me if there is any additional follow-ups or when the project is completed, so the payment can be made. I reopened my claim for the week ending 07/20/14 and got a break in form.
I tried to call Telephone Claim Center last week but they're always busy. I will try to call again, but have to fax the form back by 07/29/14 (within 7 days). So, any advice would be greatly appreciated.![]()
Please help about the following questions:
1. Name of the most recent employer: Self
Employer Registration number/ FEIN: N/A
Employer Address: My address
Is this correct?
2. Will NYS Dept of Labor send an employment verification form to me OR to the company I worked with?
Does NYS Dept of Labor usually contact by mail OR phone to verify information? What kind of information?
Anyone who got this form before, I really appreciate if you share your experience. This will help me decide if I should inform the company what to expect.
3. What is the reason of separation from this employer: Quit, Fired, Strike/Lockout, Lack of Work, Other? Since I put myself as an employer, should I answer Lack of Work OR Other?
4. I plan to attach an additional page with an explanation about whom I worked for, what I did, and my situation (similar to what I wrote above). Is the attached page necessary? Or just a brief explanation within 2 lines in the form is fine?
5. Do you have any business, or are you engaged in any other activity, which brings in or may bring in income? - No (Correct? I don't own a business. I read some people got another form asking about business they own.)
