Default Getting a Social Security Number After Changing My Last Name

    Hi,

I used to live in Kuwait ( middle east ) i never used a social security number or get a card and when i get birth they give me my mother last name because there where not married at that time ,later after my mother and father get married my dad went to health department and change my last name to his last name and this is my current name now ( he didn't go to the court at all ), so i get an amended letter from the health department ( to whom may it concern) with all information ( my old name ,my current name, date of birth , father name ,mother name and birth certificate ref number ) and they (health department) told me i can use this paper for passport and social security office.
So when i went to the SSA office they didn't accept this letter and they are asking to get a amended birth certificate same as the original birth cert with my two names ! And the health department can't do that and they are telling me this is the only paperwork they can give it to me and they keep telling me this paper is for the social security office and they should accept it.

What can i do ? Where can i get the papers so they can change my last name and use my ssn ?

Well, back up a minute. You say you used to live in Kuwait. Were you born there? If not, where were you born?

Are you a US citizen now?

if your birth certificate was amended such that you were given your father's last name, then it would have your father's last name on. If it was not amended, then whatever letter you have is meaningless. Your name was not changed legally (although that is not always a requirement)

so, give a bit of a timeline:

    how old are you

how long did you live in the states before going to Kuwait and how long since you have been back?

and since you were born here, how did you ever travel internationally without a US passport?

or, if you have a passport, is the name on it the the one using your father's last name or your mothers?

OK, well, the Department of Vital Statistics is correct, they can't provide you with what you're looking for.

You need to go to court to get a LEGAL name change, THEN, after it's granted, you can apply for an amended birth certificate. Apply for a change of name in your current locality, then after you're granted the court order, follow the instructions here.
