    Default Changing a Child's Name on a Social Security Card

    my daughters dad was in afganistan when she was born when he got back we had to fill out a form to change her last name for her birth certificate but her father lost the form that we had to sign....... what would i need to change her last name on her card?

    You either changed her surname or you didn't. If you did, then the birth certificate has been changed and you have proof of the new name to use in updating the Social Security card. If you didn't, then the father needs to get another copy of the form, complete it and file it to correct the child's name, and you can move forward from there. Name change procedures vary by state.
