Generally, phased-in. Shorter, regular visits eventually leading up to the "norm".
I agree. The only exception I have ever seen to this is if the children are teens and adamantly opposed to visiting. I have seen a few judges say that after that much time, they were going to leave it up to the teenagers. If there are multiple children and its been 13 years, everybody is likely a teenager at this point.
You are correct. The children are worried they will be forced to see him after all this time.
