Default Cancellation of Health Insurance After Support is Calculated

My question involves child support in the State of: Missouri
How does a custodial parent who is required to (and does have) health insurance on the child prove the non-custodial parent who has obtained health insurance voluntarily has cancelled the policy after the support order is modified...and further...how often can modifications be submitted?

It shouldn't be difficult to establish whether or not a policy is in effect. If the custodial parent alleges that the policy has been canceled and the non-custodial parent claims otherwise, the non-custodial parent could be asked to document that it remains in effect. Also, the custodial parent would likely have had some experience that caused them to learn of the cancellation, such as the denial of coverage under the policy after medical treatment. Otherwise, how would they know?
