Yes, you have to account for the distributions on form 8889 or the distributions will be counted as taxable income. You have to check the box in part one to indicate whether is a single or family plan. If your employer made no contributions either then that is the only part of Part I that you have to fill out. If your employer made contributions then you have to fill out the rest.

Review the instructions for the form, carefully.
