    Default Completing Tax Form 8889 for a HSA

    My question involves personal finance in the State of: Do i have to fill out part 1 of form 8889 health savings account if i did not make any contributions for 2013. I only had distributions?

    Yes, you have to account for the distributions on form 8889 or the distributions will be counted as taxable income. You have to check the box in part one to indicate whether is a single or family plan. If your employer made no contributions either then that is the only part of Part I that you have to fill out. If your employer made contributions then you have to fill out the rest.

    Review the instructions for the form, carefully.

    Default How to Complete Form 8889 for Health Savings Accounts

    Do I have to mark the single or family box in part I of form 8889 if I did not have a HDHP HSA to contribute to in 2013? I only used the remaining balance of my HSA from 2012 employer for distributions in 2013, but did not remain eligible individual for 2013 with an active HDHP HSA to contribute into. so do i have to mark one of the boxes for the HSA i took distributions from as either single or family or do i leave that blank and go to part II?

    Yes, you still have to mark either the single or family box in Part I.

    If you haven't studied Publication 969 I suggest you sit down and study it carefully before you go any further with that form.

    I am familiar with and read and study pub. 969 and the instruction form for 8889 every year i do my own taxes (and receive my refund check) nowhere in both of them does it tell you what to mark in the self or family boxes if you failed to remain an eligible individual and did not have a HDHP HSA for 2013. It only gives you instructions on what box to mark if you're single or have dependents for the family option. but it sounds like from the responses I have to still mark the self box in part I if i did not remain eligible individual and did not have an HDHP HSA in 2013 to contribute to and in 2013 only took distributions from my leftover 2012 HDHP HSA for qualified medical expenses.

    Correct...you mark the box for a an individual plan if the plan was individual to start with. Its still an individual plan until the rollover money is used up.
