It's not likely that the custodian of the account would discuss it with you since it's not your account.
That means your husband would have to contact the custodian to get a sample but the two of you can sit down and draft it together.
If the custodian declines to provide a sample you can probably find a sample on the internet or at a local law library, draft it the way you want it, and your husband can submit it to the custodian to determine if the format is acceptable.
Then you can amend the divorce papers to get the QDRO included and signed by the judge.
Just to educate you a bit, the way a 401(k) works is that the custodian of the account typically sets up a non-employee account with your share. Once that's done, you can open an IRA account and roll the funds into the IRA.
