Default How to Change My Child's Last Name to My New Husband's

My question involves name change laws in the State of: Kansas
My daughter was born in the state of Washington. Her father still lives in Washington state. Her father is on the birth certificate. We were married after her birth briefly. There is a permeant protection order in place (for my daughter & myself). Last time he saw her was over 12 years ago. I very infrequently receive child support. He has bench warrant out due to missing court dates so I have no way of contacting him. My daughter has never had his last name (always had my last name). I am remarrying at the end of this month and would like to change her name to my husbands. What do I need to do to make this happen? Do I still need permission from her father?
