    Unhappy My Current Legal Name for Years is Different from Birth Certificate Name

    My question involves name change laws in the State of: Florida
    Making up names here.
    Say my OH 1967 birth certificate name is Lisa May Jones. My mom divorces and remarries Mr. Smith and hence that last name is used from then on I am Lisa May Smith.
    In 1987 I change my first name by usage in CA, no court documents I change my first name to Lila, no more Lisa May. Now it is Lila Smith.
    In 1994 in CA I get married and my new last name from my husband is Patil so from then on, my passport, DL, ss card, everything legal name is Lila Patil.
    Only our marriage certificate and birth certificate have the original name of Lisa May Jones.
    Now I am applying for visa and they will not give because my birth certificate/marriage license name is not the same as my current name and they say I need a court document to show my name change.
    The name change form in FL is $400 + and the form says
    My legal name is ______ and I would like to change it to ______
    Do I say my legal name is Lila Patil and I would like to change it to Lila Patil? Do I have to do this I just looked at the form and it looks like the biggest hassle to fill out. Is there another way around this? TIA for any help

    It does not appear your name was legally changed from Jones to Smith. The validity of any documents going forward is questionable.

    so if you were me would you just change my name back to the birth certificate name and go back to dmv and apply for new DL, and then new passport ect... et.. ect...

    It appears you need to get the documents changed to your correct name which is either Lisa May Jones or Lisa May Patil.

    Ok thanks, for that I wouldn't need to document name change through court right? I will use Lisa May Patil if that is the case, I appreciate your replies, they have saved me much mental distress this evening, hopefully changing the documents won't be as much a hassle as filing court name change would be, I will llook inot it at the dmv and passport website
