Ex Wife Has Never Payed Child Support

My question involves child support in the State of: Texas
My ex-wife has never paid child support. Our divorce was final around July 3rd, 2006. Shortly after our divorce she landed in jail for felony drug violations. The reason for our divorce was me discovering her drug use. Also a major factor in the custody decision. She bounced around from house to house, and in and out of jail after the divorce. In my custody orders it says she is to pay child support, but I have never seen any. There was a form, that I was given, at the time of the divorce. It was never filled out, because she had no verifiable address. Could that be what stopped the process? If so, what are my options. She is semi holding down a job now and has been out of jail for about a 2 years. Its time for her to settle up, in my opinion. If the shoe where on the other foot, I could only imagine. Thanks for any advice.

If the paperwork wasn't sent, it looks like Mom isn't actually ordered to pay cs.

What was the form you were given?

Don't remember. It is now lost. There was no way to fill it out at the time, as her address was everywhere. She was a bad, meth abuser.
